What a case report needs
The CARE guideline is the standard most journals point to. It comes with a checklist you can submit alongside the manuscript.
The CARE sections
CARE sets out what a complete case report contains. Journals may merge or rename some sections, so check the author guidelines.
| Section | What it covers |
|---|---|
| Title, keywords, abstract | The words "case report", the main finding and the lesson |
| Introduction | Why the case is worth reporting, with brief background |
| Patient information | De-identified demographics, symptoms, relevant history |
| Clinical findings and timeline | Examination findings and the sequence of events |
| Diagnostic assessment | Tests, reasoning, differential diagnoses |
| Therapeutic intervention | What was given or done, doses and duration |
| Follow-up and outcomes | What happened, including adverse events |
| Discussion | Strengths, limits, comparison with literature, the take-away |
| Patient perspective and consent | The patient's view where possible; confirmation of consent |
What makes a case worth publishing
An unusual presentation of a common disease. A rare condition with a useful diagnostic lesson. An unexpected adverse effect or drug interaction. A new approach to management. A case that challenges an assumption.
Write the teaching point in one sentence before you start. If you can't, the case may suit a conference poster or a teaching session better than a journal.
Consent and confidentiality
Journals require the patient's written informed consent for publication, or that of a guardian or next of kin where appropriate, usually on a specific form. Some also ask for ethics committee confirmation.
Anonymize thoroughly: no names, initials, exact dates, locations or record numbers, and crop or mask images. Removing the name is not enough if the combination of details identifies the person.
Choosing a journal
Check that your target journal still accepts case reports; many general journals no longer do, and dedicated case-report journals exist in most specialties. Look at the article processing charge and the indexing before you commit.
